At least three weeks prior to the workshop/conference:
1. Complete the Professional Development Request Form below
2. Obtain your Principal’s signature.
3. Send completed form to District 96 Assistant Superintendent for Leadership Team review.
4. You will be notified by email upon approval.
Within two weeks after the workshop/conference:
1. Complete the Employee Expense Reimbursement Form below.
2. Attach all original itemized receipts
(not just the credit card slip - alcoholic beverages are not reimbursable)
3. Obtain your Principal’s signature
4. Send to Angela Dolezal for approval
